To deliver supports safely and effectively, Impactful Solutions collects personal and, where required, sensitive health information.
This may include information you provide directly, as well as information from:
medical professionals
allied health practitioners
schools or education providers
support coordinators
other relevant organisations involved in your supports
We will only collect information from third parties with your informed consent, unless authorised or required by law.
Providing accurate and complete information helps us deliver safe, high-quality supports. If you choose not to provide requested information:
we may be unable to provide some or all of the supports you are seeking
delivery of supports may be delayed or limited
All personal information is protected under the Privacy Act 1988 (Cth). You have the right to access the personal information we hold about you and to request corrections if it is inaccurate or out of date.
Use and Disclosure of Your Personal Information
We use your personal information to:
determine whether we can provide the supports you are seeking
deliver, monitor, and review your NDIS-funded supports
develop, implement, and review behaviour support and developmental education plans
collaborate with your consented support network
support you to access services outside the NDIS, where requested
We do not use your personal information for any other purpose, nor disclose it to other organisations or individuals (including overseas entities), unless:
you have provided informed consent
we are authorised or required by law
Storage and Security of Your Personal Information
Impactful Solutions stores personal information using secure electronic systems and platforms that meet privacy and security requirements.
Access to personal information is restricted to authorised workers only, and staff are required to:
use information only for work-related purposes
maintain confidentiality at all times
comply with internal privacy, record-keeping, and cybersecurity procedures
